Medication Log Tips
Not medical advice. This guide is general information for personal record keeping. Always follow your doctor's or pharmacist's instructions.
An up-to-date medicine list and dose log can make doctor visits, pharmacy trips and emergencies easier for you and your family.
Keep one complete list
- Write down every medicine, including over-the-counter medicines, vitamins and supplements.
- For each one, note the name, strength, how much you take, when, and what it is for.
- Add the prescribing doctor and pharmacy.
- Record allergies and any reactions you have had.
Track doses day to day
- Check off each dose when you take it, so you are not left wondering.
- Note missed doses and anything unusual you notice.
- Keep the log in the same spot, near where you keep your medicines.
Update and share
Update your list whenever a medicine is started, stopped or changed. Bring it to every appointment and ask your doctor or pharmacist any questions about your medicines. Never change how you take a medicine without talking to them first.
